Franklin Youth Hockey Coach Registration

Application Instructions:

To apply to coach in the Franklin Youth Hockey Association for the 2007-2008 season please do the following:

1. Download the FYHA Coaching Package - by clicking on its link.
   
   
2. Read the Liability Waiver. BY YOUR SIGNATURE ON THE COACH'S APPLICATION YOU SIGNIFY THAT YOU AGREE WITH AND WILL ABIDE BY ALL ITEMS in the FYHA By-Laws, the USA Hockey Coaching Ethics Guide and the Liability Waiver.
   
   
3. Fill out the Application form. Don't forget your signature, email address and the date.
   
   
4. Mail the Application form to:

   Franklin, MA 02038
